PD-L1-expressie als biomarker bij longkanker
PD-L1-expressie, bepaald met immunohistochemie (IHC) op tumorweefsel, is de meest gebruikte biomarker voor selectie van immunotherapie bij NSCLC. De Tumor Proportion Score (TPS) meet het percentage PD-L1-positieve tumorcellen. Bij TPS ≥50% is pembrolizumab monotherapie een optie; bij TPS 1-49% wordt combinatie met chemotherapie aanbevolen.

PD-L1 als immunotherapie-biomarker
Klinische toepassing
TPS ≥50%: pembrolizumab monotherapie (KEYNOTE-024) of combinatie met chemotherapie. TPS 1-49%: chemotherapie plus pembrolizumab (KEYNOTE-189/407). TPS <1%: chemotherapie plus nivolumab-ipilimumab (CheckMate 227/9LA) of chemo-immunotherapie. PD-L1-negatieve tumoren met hoge TMB kunnen ook profiteren van immunotherapie.
