Durvalumab (Imfinzi)
Durvalumab is een anti-PD-L1-antilichaam met twee belangrijke indicaties in de thoracale oncologie: consolidatietherapie na gelijktijdige chemoradiotherapie bij onresectabel stadium III NSCLC (PACIFIC) en eerstelijns gecombineerd met chemotherapie bij extensive-stage SCLC (CASPIAN).

Durvalumab bij thoracale tumoren
Stadium III NSCLC
PACIFIC: na gelijktijdige chemoradiotherapie wordt durvalumab gedurende 12 maanden als consolidatie gegeven. 5-jaars OS 43%. PD-L1-expressie ≥1% wordt door sommige richtlijnen als voorwaarde gesteld. Dit regime is de standaard bij onresectabel stadium III NSCLC.
ED-SCLC
CASPIAN: durvalumab plus carboplatine-etoposide als eerstelijns: mediane OS 13.0 vs 10.3 maanden. Toevoeging van tremelimumab voegde geen significant voordeel toe.
