R-CHOP-chemotherapie
R-CHOP (rituximab, cyclofosfamide, doxorubicine, vincristine, prednisolon) is al meer dan 20 jaar de standaard eerstelijnsbehandeling voor DLBCL. 6-8 cycli elke 21 dagen. Curatie bij ~60%. Pola-R-CHP (polatuzumab vedotin vervangt vincristine) verbetert PFS bij hoogrisico-DLBCL (POLARIX).

R-CHOP: rituximab 375 mg/m² dag 1, cyclofosfamide 750 mg/m² dag 1, doxorubicine 50 mg/m² dag 1, vincristine 1.4 mg/m² (max 2 mg) dag 1, prednisolon 100 mg dag 1-5. Cyclus elke 21 dagen, 6-8 cycli. Standaard eerstelijns bij DLBCL sinds >20 jaar. Pola-R-CHP: polatuzumab vedotin vervangt vincristine — superieure PFS bij IPI ≥2 (POLARIX). G-CSF profylaxe bij >20% risico op febriele neutropenie.
