Totale neoadjuvante therapie (TNT) combineert alle systemische therapie en radiotherapie vóór chirurgie bij lokaal gevorderd rectumcarcinoom. Het TNT-concept verbetert de compliance met systemische therapie, verhoogt het percentage complete pathologische responsen en opent de deur naar watch-and-wait bij klinisch complete respons.

Schema's
RAPIDO: 5×5 Gy gevolgd door 6 cycli CAPOX, dan chirurgie. PRODIGE-23: FOLFIRINOX inductie gevolgd door chemoradiotherapie en chirurgie. Beide schema's tonen meer pCR en minder afstandsmetastasen dan conventionele chemoradiotherapie gevolgd door chirurgie en adjuvante chemotherapie.
