Reactie op brief over PET/CT-gestuurd management van checkpointblokkade bij longkanker

We thank Dr Zhang and colleagues for their interest in our work, ‘PET/CT-Guided Management of Immune Checkpoint Blockade and Post-Treatment Multi-Modal Profiling in Long-Term Responders with Metastatic Lung Cancer in the National Network Genomic Medicine Lung Cancer Germany (nNGM)’.1 The letter raises concerns regarding the statistical handling of the dynamic cohort, asymmetries in the definition of progression-free survival (PFS), potential confounding by intensified local ablative therapy and biopsy-driven surveillance, and insufficient adjustment for depth of response, questioning whether the observed survival differences can be attributed to the discontinuation strategy itself.
