The integration of immune checkpoint inhibitors (ICIs) has transformed outcomes for patients with stage II-III triple-negative breast cancer (TNBC). The landmark KEYNOTE-522 (KN-522) trial demonstrated improvements in pathological complete response (pCR), event-free survival (EFS), and overall survival (OS),1 similar to other neoadjuvant ICI studies (Table 1).1-6 Despite these advances, global access to ICIs remains limited. High out-of-pocket drug costs,7,8 long infusion times, and management of serious life-altering/threatening immune-related adverse events (irAEs) contribute to financial hardship and increased health care utilization, especially in low- and middle-income countries.
